St. Luke's Health System in Boise is seeking a Full-Time Valve Nurse Coordinator/Navigator to join our SL Idaho Cardiology Associates Clinic - Structural Heart & General Cardiology team.

At St. Luke's Clinic - Idaho Cardiology Associates, the team has over 150 years of combined experience in the treatment of heart disease and offers you comprehensive cardiovascular care. Clinic services include electrophysiology and arrhythmia exams and consultations, transthoracic and transesophageal echocardiograms, stress tests, and nuclear cardiology.

This position will monitor, manage and oversee the still growing structural heart valve program within St. Luke's Idaho Cardiology Boise office in conjunction with our cardiology providers. The Nurse Navigator will serve as the valve transplant coordinator within the structural heart team and will assist with patient care and the scheduling of procedures involved with the management of this patient population.

Further Responsibilities Include:

• Coordinates patient care from the time of referral to the time of implant, and helps navigate the patient & family through the referral and screening process.
• Provides patient education regarding the referral, procedural, and post-procedural process; education also to include cardiovascular conditions and treatment options as directed by the cardiologist.
• Triages patients' physical symptoms / arrhythmias, whether in person or via phone call, appropriately and consults with cardiologists per protocol.
• Serves as primary patient point of contact for phone calls and will assure all screening appointments have been ordered & manage appropriately.
• Participates in Transition of Care Management to ensure discharge medications and instructions are understood by patient and/or family members.
• Tracks, maintains, and monitors the valve patient population using Microsoft Excel and TEAMS, and helps maintains compliance documentation for insurance and the NCDR.
• Collaborates with TVT registry abstractors to ensure data integrity.
• Reviews physician tasks and clinical worklist(s) as assigned.
• Reviews preoperative lab and imaging results when applicable and ensures appropriate management of results with structural heart providers.
• Assists in matters relating to nursing practices and participates in development of policies and protocols for structural heart patient care.
• Promotes ongoing communication and collaboration with the clinical manager, nursing supervisor, physicians, and appropriate staff to ensure patient safety and adherence to clinic protocols.
• Works in collaboration with structural NPs and Cardiologists to review data for related improvement projects.
• Other duties as assigned.

(4) nine hours shifts, Monday - Friday, flexible schedule.

Responsibilities

• Serves as a single point of contact for patients undergoing treatment,.
• Serves as a liaison between the patient and their family and the patient's network of care providers.
• Educates patient on diagnosis, treatment options, and prognosis.
• Assists the patient in scheduling appointments, completing paperwork, or securing referrals.
• Facilitates short term and/or long term interventions.
• Possesses comprehensive knowledge of subject matter.
• Handles complex issues and problems, and refers only the most complex issues to higher-level staff.
• Other duties and responsibilities as assigned.

Qualifications

• Education: Bachelors of Nursing Degree
• Experience: 2 years's of RN experience
• Licenses/Certifications: Current RN License in the State of Practice
