The role you’ll contribute:
Provides overall coordination of the Structural Heart program for Advent Health Daytona Beach. Provides care under the direction of the supervising Cardiologists and Cardiovascular Surgeons. Has expertise in the clinical management of the Cardiac Surgical/Procedural patient. The Structural Heart Coordinator has the responsibility for assessment and management of cardiac patients including diagnostic and therapeutic intervention, day to day implementation and evaluation of the appropriate plan of care. The Structural Heart Coordinator assists the CV Surgeons and Cardiologists with rounds, creation of orders (admission, ongoing, and discharge), monitoring of tests, and assessment and evaluation at the bedside. The Structural Heart Coordinator works in collaboration with the medical, nursing and support services to provide patient and family focused care. The Structural Heart Coordinator maintains active participation in PI activities, STS Registry data collection, staff development and continuing education.

- Implements Structural Heart Program's processes and protocols to assure smooth operations and patient flow.
- Follows consistent screening process for all Structural Heart patients referred to the center in order to triage patients to the
- appropriate therapeutic pathways.
- Manage all incoming referrals to the Structural Heart Center including but not limited to:
- Perform initial demographic and clinical evaluations on patients referred with structural heart disease.
- Schedule, coordinate all appropriate screening tests (i.e., laboratory, radiology, echo, PFT and other studies) in
- collaboration with Structural Heart Team.
- Complete initial patient assessment including frailty testing, Cardiomyopathy questionnaire, Katz Index of Independence
- in Activities of Daily Living and STS risk assessment.
- Review medications in accordance with guidelines developed with supervising physicians for pre-testing, pre-operatively,
- and post-surgical.
- Compile full screening portfolio including PowerPoint or Word presentation for each screened patient for physician team
- review at the weekly valve conferences.
- Initiate consultations/referrals as dictated by physician team.
- Schedule procedures.
- Ensure consistent and appropriate follow-up care in collaboration with referring and or primary care physicians.
- Manage the full patient experience from referral to procedure. Including but not limited to
- Educate the patient and family on the referral and screening process for the Structural Heart Program.
- Educate the patient and family on the potential options given their disease state.
- Maintain consistent communication with the patient and family throughout the screening, pre-procedure process, post[1]procedure during hospitalization.
- Daily rounds for hospitalized patients with documentation in the progress notes.
- Pre and port procedural NIH Stroke Scale, KATZ scoring, and CQ questionnaire.
- Interpret all appropriate laboratory, radiology tests during hospitalization in collaboration with Structural Heart team.
- Follow up with patient via phone post procedure to check in on status and needs.
- Promote understanding of the underlying disease process and care planning.
- Work collaboratively with the vendors for the Structural Heart program to ensure we are compliant with all requirements.
- Obtain images from internal/external IT system.
- Complete and load required patient summary/PowerPoint.
- Discuss upcoming cases with ELS vendors.
- Complete all documentation to ensure compliance with regulatory standards related to quality and billing (i.e., complete
- TAVR/LAAO registry data collection and entry for all TAVR/LAAO patients, etc.)
- Collaborate with Clinical Documentation Specialist to assure all documentation and billing requirements are met per CMS
- and all regulatory guidelines.
- Ensure uniform and comprehensive clinical data collection per CMS and all regulatory guidelines.
- Collaborates with Outcomes Management Department to insure that TAVR registry data elements are evident in the chart.
- Collaborate with CVI Research division and Structural Heart research.
- Collaborate with the physician team to develop short and long range goals for the Structural Heart Program.
- Complete the grid for the weekly Case Review conference.
- Represent the AH Daytona Beach Structural Heart program at community outreach programs, including evening
- programs.
- Cultivate existing referral base while establishing new referral lines by working with the physician team to visit primary
- care and cardiology practices routinely.
- Work with the team and the marketing department to develop educational materials for the Structural Heart Program.
- Provide Phone coverage of the Structural Heart line and Structural Clinic on-line mailbox.
- Assist in covering cardiovascular services and functions as needed.
- Responsible for ensuring database is completed accurately and timely.
- Assists with STS data collection.
- Makes daily rounds on Structural Heart patients and reports findings to the attending Physician. Manages patients,
- evaluates changes in patient status, obtains consultations, test and X-ray results. Gives discharge instructions.
- Schedules procedures, lab work and medications.
- Refers patient for specialized consultations.
- Initiates treatment of emergency management and patient resuscitation.
- Facilitates follow up patient care.
- Utilizes his/her knowledge and skills in giving safe and effective patient care and teaching.
- Schedules and monitors tests and other diagnostic procedures.
- Participates in hospital/community functions.
- Maintains working knowledge of TJC/CMS/AHCA and Core Measures.
- Consistently complies with documentation standards and regulatory body requirements (ie. CMS, JC, STS, ACC, etc).
- Works with Case Management to assure appropriate utilization of services and length of stay for cardiac patients.

QualificationsKNOWLEDGE AND EXPERIENCE STRONGLY PREFERRED:
Good computer skills required
Knowledge of adult learning principals
Demonstrated skill in the use of written and oral communication
KNOWLEDGE AND EXPERIENCE PREFERRED:
Ability to write objectives, content outlines, and bibliographies desirable.
EDUCATION AND EXPERIENCE REQUIRED:
Bachelor’s Degree in nursing
Minimum of 3 years' Nursing experience in Cardiovascular Services.
EDUCATION AND EXPERIENCE PREFERRED:
Past teaching experience in formal/informal settings preferred
5 - 10 years experience in Critical Care Nursing, Cath Lab, or CVOR
Structural heart (TAVR, Watchman, mitral clip) experience – a plus!
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
• AHA Advanced Cardiac Life Support Cert.
• AHA Basic Life Support Cert.
• IL Registered Nurse License
